RN - Family Case Management

Southern Seven Health Department is seeking applications for a regular, full time RN position that is based at the Massac County Public Health Clinic in Metropolis, Illinois.  This position will also provide case management services at another S7HD clinic but will receive mileage reimbursement.  It does come with a benefit package, including vacation, personal, and sick time accrual, holidays, health and life insurance, and an employer sponsored cafeteria plan.  This position performs core case management activities of assessment, planning, referral, advocacy, and follow-up.

Qualifications:  The applicant must have a valid Illinois RN license; preferences will be given to an applicant with previous experience in case management activities.

Job Duties:

• Provides case management services to assigned pregnant women, infants and at risk infants, and children.
• Completes a comprehensive needs assessment and individualized care plan on all families enrolled within a required time frame.
• Performs home visits on all at risk case managed clients and provides appropriate referrals as needed.
• Provides face to face contacts on all case managed clients and provides appropriate referrals as needed.
• Performs screening on all women for post-partum mood disorders and refers to services as appropriate.
• Performs Well Child/EPSDT exams on case managed infants and at risk children as per policies and procedures.
• Provides keyboard data entry of client data and medical care coordination into online data base program.
• Maintains client files including filing duties.
• Follows all policies and procedures related to confidentiality and HIPAA regulations.
• Performs other duties as requested or assigned to meet the requirements of the agency.
