Southern Seven Health Department is seeking applications for a regular, full time LPN position that is based at the Pulaski County Public Health Clinic in Ullin, Illinois but would require travel among all Southern Seven counties. This position does come with a benefit package, including vacation, personal, and sick time accrual, holidays, health and life insurance, and an employer sponsored fringe benefit plan. This position is primarily responsible for assisting various services at Wellness on Wheels events.

Qualifications: The applicant must have graduated from an accredited Practical Nursing program and have an up-to-date LPN license in IL.

Job Duties:

• Maintains confidentiality of all client records.
• Assures documentation of service delivery in accordance with professional standards and Southern Seven policies and procedures.
• Performs home visits according to agency and Southern Seven policy and procedures.
• Coordinates client activities and referrals within the agency across programs as well as with school and other community agencies as appropriate
• Maintains knowledge of local and state program policies and procedures.
• Participates in communicable and chronic disease investigation and surveillance such as STD programs.
• Performs delegated medical functions such as vital signs, heights/weights, TB skin tests, pregnancy tests, blood lead tests, hemoglobin and cholesterol screenings, STD testing, etc.
• Prepares and administers vaccines for an immunization in accordance with Southern Seven policies and procedures and standing orders.
