Contact Tracer Resource Coordinator

Southern Seven Health Department is seeking applications for a regular, full time  position as Contact Tracer Resources Coordinator that is based at the Rustic Campus in Ullin, Illinois. This position does come with a benefit package, including vacation, personal, and sick time accrual, holidays, health and life insurance, and an employer sponsored cafeteria plan.  This position is responsible for assisting COVID-19 diagnosed patients with getting needed resources.

Qualifications: Bachelor's Degree in Social Work, Public health or related degree  OR 4 years of related professional experience.

Job Duties:

• Communicating effectively with newly COVID-19 diagnosed patients.
• Identify services and resources that would benefit COVID-19 diagnosed patients.
• Resolve issues and implement solutions.
• Coordinate delivery of needed resources to COVID-19 patients and their contacts during isolation/quarantine period
• Participates in all required trainings.
• Enters data into appropriate computer software systems as required.
• Follows all policies and procedures related to confidentiality and HIPAA regulations.
• Performs other duties as requested or assigned to meet the requirements of the agency.
• Assist with contact tracing investigations when needed.
